—Applicant’s Photo ID
—Proof of Income for each adult member of the household excluding dependents who are full-time students (monthly paystuds, SSD / SSI / SS award letter, bank statements, unemployment, retirement, child support, letter from employer showing amount earned in period covered [if cash-based]). The Applicant’s current household income and income prior to March 1, 2020, must not exceed 60% of the HUD 2020 Atlanta-Sandy Springs-Roswell, Georgia Metro Area Median Income.
—Proof of residence (Lease)
—Proof of last two rental payments or landlord ledger of payments (e.g. bank draft, receipt, cancelled check, etc.) OR current mortgage statement
—Proof of Extenuating Circumstance (e.g. separation / furloughed notice reduced hours, check stubs showing work hours before and after the onset of the pandemic, verification of sick leave, FMLA, a copy of utility bills showing an increase due to the pandemic, copies of medical bills incurred after the onset of the pandemic, etc.)
—Bill – Current or Past Due (Late or Eviction Notice, Rent Statement, Gas, Electric)
